PURPOSE OF THIS ROLE
The Community Tracer is responsible for locating identified HIV infected individuals who need to be linked to health care facilities for further treatment. They will also be responsible to provide education and information around treatment and care of HIV and or TB.

Technical Work Responsibilities
Coordinate with healthcare facilities to locate identified HIV- positive clients /cases requiring linkage back to care
Perform intake needs assessments on all assigned clients to determine the level of support required for each client
Review care plans collaboratively with the patient and other members of the support team and provide relevant advise
Coordinate referrals to clinic and community-based HIV care and treatment services, and advocate for accessibility to these services
Conduct follow-up visits to deliver case management support to assigned patients as per case management protocol
Submit dispatch files to the sub-district utilising the relevant established channels
Liaise with patient navigators and data monitors at facility level to confirm patients are linked to care utilising Tier.net
Liaise with case officers on the lost cases from the CBO’s for a further follow-up to their households.
Ensure appropriate recording and reporting of all data obtained in the field
Find more effective and efficient ways of doing ones work
Tracing Related Tasks
Link clients to the relevant HIV care and treatment services
Coordinate referrals to clinic and community-based HIV care and treatment services

Assist clients with the completion of forms
Review care plans
Conduct follow-up visits in-person to deliver case management support to assigned clients
Submit review, activity and all other reports when required
Manage client records
Submit dispatch files to the sub-district
Consult with patient navigators and data monitors at facility level to confirm patients are linked to care using Tier.net
Consistently re-assuring clients of the confidentiality of their status and other sensitive information
Requirements
YOU MUST HAVE:
Qualification/Professional Membership
Grade 12
Basic knowledge / education of HIV / AIDS, STI’s and TB
Advantageous
HIV/AIDS and TB Counselling Certificate
Work Proven Experience
At least 6 months’ experience in a field work, health promoter gained through formal employment / volunteering work
Other requirements
Excellent social kills, team player, performance oriented and self-motivated
Excellent communication skills and the ability to establish effective, trusting relationships
Demonstrable empathy, compassion and an understanding of the cultural and socioeconomic backgrounds in the community
Willing to travel within / outside of the local working area
Excellent knowledge of the local catchment area
